Looks like a software error should take most of the blame. Getting a single digit wrong happens regularly, software should be able to handle that. It should verify the data entered and give reasonable opportunities to correct errors, even after the flight has started. I know that lots of planes are old, a 50 year service life isn't uncommon. But updating the electronics is a fairly standard change. Yes, not quite the hapless screwup I'd assumed based on merely skimming the headline. (I had just woken up, saw the headline, and misread it as stating that the pilot truly thought he had reached Malaysia.) The linked _SMH_ had some more information -- though, oddly, not the flight number or exact airplane type. The picture makes it look like an A320. Well, at least the good news is that AirAsia X's pilots are still trained well enough to do VOR approaches and landings. ;->
