Looks like a software error should take most of the blame. Getting a single digit wrong happens regularly, software should be able to handle that. It should verify the data entered and give reasonable opportunities to correct errors, even after the flight has started. I know that lots of planes are old, a 50 year service life isn't uncommon. But updating the electronics is a fairly standard change.
